If you choose to submit your payment via credit card, payment would need to be finalized 4 days prior to the exam.The merchants website will indicate that we will ship you your kit, we will not ship you a kit. Your kit will be located at Carrington College in Pomona. Day of the Exam, please remember to bring your cleaning deposit in envelope. | When you rent a Kit, please arrive 2 hours early so that you can review what is in your kit. The Board will not help you with any of your equipment during the examination. It is your responsibility to make sure you know how to operate all equipment and to make sure everything is stocked and ready to go. Thank you for Choosing our company to assist you. |

There is a $35.00 cleaning deposit. It is fully refundable based on the condition of the returned kit. Please bring this deposit with you to the Exam Site, in cash, in an envelope with your name on it. Thank you! |
| Payments: Make Money Order/Personal check payable to "J and Z Dental Rentals" and mail to 27201 Valleymont Road, Lake Forest, CA 92630. This form of payment must be received 8 days prior to State Board, unless other arrangements have been made. We also accept credit card Visa, M/C, Discover Card, American Express via PayPal on our Website. Cancellation and Refund Policy: Refunds will be made only with a 5 day notice prior to the State Board. No Show policy: No show no refund: If you pay with a debit card or a credit card , we will issue a refund to you, for the net amount that was issued to J and Z Dental Rentals & Clinical Seminars from the credit card merchant, and will need to send you the refund by mail. We are sorry for this inconvience. Seminar Policys please refer to the appropriate page. All Combo package Seminars need to be completed with in 6 months from the date entered on the registration form.
